Please complete this form if your school is interested in participating in the 3rd Annual Back To School With The HistoryMakers program on Friday, September 28, 2012. Be as thorough as possible as the requested information is critical to the planning and execution of the program. Also, be sure to check your school's calendar first to make sure there are no scheduling conflicts.
